Study shows bariatric surgery improves, reverses diabetes

Overweight, diabetic patients who underwent bariatric surgery achieved significant improvement or remission of their diabetes, according to new research from Cleveland Clinic.

In a randomized, controlled trial, some weight loss surgery patients achieved normal blood sugar levels without use of any diabetes medications. In others, the need for insulin to control blood sugar was eliminated. Recent observational studies had demonstrated that bariatric surgical procedures reduce the incidence of type 2 diabetes and lead to substantial improvement for many patients with pre-existing disease.

"After one year, patients who underwent gastric bypass or sleeve gastrectomy lost more weight and were significantly more successful at controlling their diabetes, compared to those who simply took medications," said lead investigator Philip Schauer, M.D., Director of the Cleveland Clinic Bariatric and Metabolic Institute. "We believe that bariatric surgery represents a potentially valuable strategy for control of diabetes that should be considered in more patients who do not respond to conventional treatment."

Results of the STAMPEDE (Surgical Therapy And Medications Potentially Eradicate Diabetes Efficiently) trial were published today in The New England Journal of Medicine and presented today at the Annual Scientific Sessions of the American College of Cardiology in Chicago.

The STAMPEDE trial involved 150 patients with obesity and poorly controlled diabetes. The patients were divided into three groups of 50: those who received intensive medical therapy of their diabetes, those who received intensive medical therapy plus gastric bypass surgery, and those who received intensive medical therapy plus sleeve gastrectomy.

"This trial demonstrates that bariatric surgery can eliminate the need for diabetes medications in many obese patients whose diabetes is poorly controlled," Schauer said. "Furthermore, the surgical patients showed major improvements in other measures of heart health, including reduced need for high blood pressure and cholesterol medications, while significantly boosting HDL ? the so-called 'good' ? cholesterol."

Effectiveness was gauged by the percentage of patients who achieved an average blood sugar within the normal range 12 months after treatment (measured using the hemoglobin A1c ? HbA1c ? a standard laboratory test that reflects average blood sugar over three months).

After 12 months, a normal HbA1c (less than 6.0) was achieved in 42.6 percent of patients who underwent gastric bypass and 36.7 percent of patients who underwent sleeve surgery, but just 12.2 percent of medically treated patients. The HbA1c of less than six is a more aggressive target than the American Diabetes Association guidelines.

Participants entered the study taking an average of three medications each for diabetes. In all of the gastric bypass patients who achieved the target level, the normal blood sugar was attained without use of any diabetic medications. Seventy-two percent of sleeve patients who reached normal blood sugar also did so without the use of any diabetic medications. In surgically-treated patients who continued to require drugs, researchers observed a substantial reduction in the need for diabetic medications.

As expected, the patients who received bariatric surgery lost more weight during the 12-month study, averaging 64.7 pounds for patients who received gastric bypass, 55.2 pounds for patients who had stomach reduction surgery, and 11.9 pounds for patients treated with medications.

The study authors reported some complications of surgery, but most were not serious. However, four patients did require a second operation. The study authors caution that the favorable results were observed after a relatively short follow-up period (12 months) and that long-term studies are needed to determine the durability of the findings. The authors will continue to follow these patients for four years to attempt to answer these questions.

You never know what a DNA test might reveal!

A few months ago, spurred on by my interest in genetic genealogy, my sister Erin decided to order a DNA test from 23andMe for my brother-in-law John Huffer. Since John has never known his father, we didn?t really know what his results would show. His mother?s family has a strong oral tradition of Native American ancestry, so we expected some mixed ancestry. However, what we found was unexpected.

Each member of the group had ideas about improving the overall experience of using 23andMe, including more voices from users. Opening up the blog to guests like our Ancestry Ambassadors is one way to do that.

His Ancestry Painting, in the image below, revealed that he has five percent African ancestry.? 23andMe?s Ancestry Painting examines the 22 pairs of chromosomes one segment at a time and determines for each stretch whether it was most likely inherited from ancestors in Africa, Europe or Asia and ?paints? the segments based on that different ancestry.

I had never investigated John?s family tree before, but my curiosity was piqued. Fairly quickly it became obvious that his was no run-of-the-mill genealogy. John?s mother?s line traces straight back to Madison Hemings. That name may be familiar to you. Madison Hemings was the son of Sally Hemings, a mixed race slave of Thomas Jefferson, our third president and author of the Declaration of Independence. My genealogy research clearly showed that Thomas Jefferson and Sally are John?s fourth great-grandparents!*

John has always especially admired Thomas Jefferson above all other historical figures and felt a special affinity to Monticello when he first visited there ten years ago, although he had absolutely no idea of his familial connection. When I first relayed my findings to John and my sister, they were, understandably, shocked. Once they started browsing the Internet in search of more information, they came across a very familiar photo on one of the websites about Monticello. An original of this very photo had hung in John?s house growing up. Further investigation shows that his mother had at least a vague knowledge of her family?s relationship to Thomas Jefferson and/or Sally Hemings, but did not share it with her children.

There have been a number of interesting ?coincidences? in regard to the Huffers and Monticello that make one ponder on genetic memory. When John and my sister first visited Monticello on an anniversary trip ten years ago, they loved it so much that they decided to go back the very next year with their daughter Courtney.

When Courtney entered the dining room of Monticello, she promptly passed out. Although her reaction could be attributed to the hot, humid day, it bears notice that this was the one and only time that Courtney has ever fainted. This was long before the family knew about their personal connection to the place or the families that lived there.? Courtney says that although she felt ?overwhelmed? at Monticello, she also ?felt at home?. Recently, the family visited Monticello for the first time since discovering their relationship to it and its previous inhabitants. John and Courtney both said that Monticello ?seems like a happy place? and gave them a sense of belonging.

Since John had never had any relationship with his father?s family and had very little contact with his mother?s side after a family dispute, he says it was as if they ?had no history?. Erin relates, ?It?s been fun for John to finally have a legacy of some sort, not to mention one as interesting as this,? and goes on to say that this discovery has finally given John and his siblings ?a sense of who they are and where they came from.? John?s young nephew Joshua tells me that learning about his illustrious roots has inspired him to strive to make more of his life and given him the confidence that he can achieve anything he desires.

I am so happy for John, once a fatherless little boy, who can now take comfort and pleasure in the knowledge that he is directly descended from one of the founding fathers of our country! During his most recent visit to Monticello, he couldn?t help but wonder if his deep love of Paris is simply a coincidence or caused by the same unknown spark that made Thomas Jefferson fall in love with it so long ago. Or, if his interest in the art of brewing beer could have come from his forebearers, whom he learned on his visit were expert brewers at Monticello. It is wonderful for John and his family to finally have these types of questions about their ancestry to ponder; questions that most of us take for granted.

DNA testing has come full circle for genetic genealogy with John?s story. It is fitting that one of the very first public uses of DNA for genealogical purposes was the Y-DNA test in 1998 that originally addressed the controversy surrounding the paternity of Sally Hemings? children. Today, more than a decade later, the simple decision to test with 23andMe is what led John to make this illuminating discovery about his roots.

[*Although some Jefferson researchers had disputed the validity of the claims that Sally?s children were fathered by Thomas Jefferson, most of these doubts were put to rest after a 1998 Y-DNA test on Sally?s son Eston?s direct male line descendants proved that they carry the Y chromosome of Thomas Jefferson?s male line. Although Madison has no living direct line male descendants who could have their Y-Chromosome DNA tested to further support this claim, it is now widely accepted that Thomas Jefferson fathered all of Sally?s children. This is supported by oral histories of Sally?s descendants and analysis of the couple?s relationship and time spent together. Annette Gordon-Reed provides a detailed look at this evidence in her exhaustive study and Pulitzer Prize winning book, ?The Hemingses of Monticello?. The official Jefferson Monticello website also addresses this issue. John?s discovery has sparked my interest in this fascinating subject. As a result, I am spearheading an autosomal DNA project on the descendants of Sally Hemings. Although it is now generally recognized that Thomas Jefferson fathered all of Sally Hemings? children, it has been impossible to determine with certainty. The introduction of autosomal DNA testing has changed this. Through autosomal DNA testing of specific individuals, I hope to be able to demonstrate that the descendants of Sally Hemings share blocks of DNA with Jefferson?s ?legitimate? descendants. If it is not possible to procure DNA from any of these known descendants, I will seek DNA from descendants of individuals one step further back in Thomas Jefferson?s pedigree in an attempt to show that Sally?s descendants possess DNA from Jefferson?s ancestral lines. Since autosomal DNA undergoes random recombination with each successive generation, it will be necessary to test the oldest living descendants of these lines. TOKYO (AP) ? One of Japan's crippled nuclear reactors still has fatally high radiation levels and hardly any water to cool it, according to an internal examination Tuesday that renews doubts about the plant's stability.

A tool equipped with a tiny video camera, a thermometer, a dosimeter and a water gauge was used to assess damage inside the No. 2 reactor's containment chamber for the second time since the tsunami swept into the Fukushima Dai-ichi plant a year ago. The probe done in January failed to find the water surface and provided only images showing steam, unidentified parts and rusty metal surfaces scarred by exposure to radiation, heat and humidity.

The data collected from the probes showed the damage from the disaster was so severe, the plant operator will have to develop special equipment and technology to tolerate the harsh environment and decommission the plant, a process expected to last decades.

Tuesday's examination with an industrial endoscope detected radiation levels up to 10 times the fatal dose inside the chamber. Plant officials previously said more than half of melted fuel has breached the core and dropped to the floor of the primary containment vessel, some of it splashing against the wall or the floor.

Particles from melted fuel have probably sent radiation levels up to dangerously high 70 sieverts per hour inside the container, said Junichi Matsumoto, spokesman for Tokyo Electric Power Co.

"It's extremely high," he said, adding that an endoscope would last only 14 hours in that condition. "We have to develop equipment that can tolerate high radiation" when locating and removing melted fuel during the decommissioning.

The probe also found the containment vessel ? a beaker-shaped container enclosing the core ? had cooling water up to only 60 centimeters (2 feet) from the bottom, far below the 10 meters (yards) estimated when the government declared the plant stable in December.

Finding the water level was important to help locate damaged areas where radioactive water is escaping.

He said that the actual water level inside the chamber was way off the estimate, which had used data that turned out to be unreliable. But the results don't affect the plant's "cold shutdown status" because the water temperature was about 50 degrees Celsius (122 Fahrenheit), indicating the melted fuel is cooled.

Three Dai-ichi reactors had meltdowns, but the No. 2 reactor is the only one that has been examined because radiation levels inside the reactor building are relatively low and its container is designed with a convenient slot to send in the endoscope.

The exact conditions of the other two reactors, where hydrogen explosions damaged their buildings, are still unknown. Simulations have indicated that more fuel inside No. 1 has breached the core than the other two, but radiation at No. 3 remains the highest.

The high radiation levels inside the No. 2 reactor's chamber mean it's inaccessible to the workers, but parts of the reactor building are accessible for a few minutes at a time ? with the workers wearing full protection.

Last year's massive earthquake and a tsunami set off the worst nuclear accident since Chernobyl, sending three reactor cores to melt and causing massive radiation leaks. The government said in December that the reactors are safely cooled and the plant has stabilized, while experts have questioned its vulnerability.

During a recent visit by a group of journalists including The Associated Press, the head of the plant said it remains vulnerable to strong aftershocks and tsunami and containing contaminated water and radiation is a challenge. Radioactive water had leaked into the ocean several times already.

Workers found the fresh leak of 120 tons from a water treatment unit this week from one of its hoses, with estimated 80 liters (20 gallons) escaping into the ocean, Matsumoto said. Officials are still investigating its impact.

Fukukushima's accident has instilled public distrust and concerns about nuclear safety, making it difficult for the government to start up reactors even after regular safety checks. All but one of Japan's 54 reactors are now offline, with the last one scheduled to stop in early May.

That?s the idea being floated by Spotlight Theaters CEO Joe Paletta in a recent article appearing in Screentrade Magazine (check out the online copy here).

Paletta says that the future of movie exhibition looks bright and that big changes are on the horizon. One of the biggest, as he sees it, will be a move to eliminate the 3D movie surcharge. The increased price of a 3D ticket has been justified in several ways since Hollywood decided to wholeheartedly embrace the technology a few years ago (primarily based on the cost of keeping a supply of re-usable 3D glasses and the price of retrofitting theaters to exhibit films in the format), but Paletta thinks that?s about to change. He envisions a future where 3D and 2D ticket prices will ?blend? together ? with 3D prices coming down and 2D prices rising to make up the difference. What a glorious future for all of us!

As Roger Ebert points out, this feels a lot like punishing consumers who don?t enjoy 3D and don?t care to view films in the format.

While this plan will make 3D tickets more affordable, everyone else winds up paying more than they already were. Since 2D films are still the standard, this means most people will be paying more ? not less ? to see a film in the theater. That?s sure to get those attendance numbers up.

Of course, this is all just the speculation of one industry insider ? and maybe it won?t come to pass. One thing seems clear, though: Hollywood needs to address the issue of 3D ticket prices before patrons become frustrated with the format. This is one way to do it ? but probably not a way that?s going to sit well with most customers.

Colo. wildfire kills one; 900 homes ordered evacuated

CONIFER, Colo. -- One person was killed in a Colorado wildfire that burned more than four-and-a-half square miles and destroyed at least five homes in the mountains southwest of Denver, authorities said Monday.

The victim's name wasn't immediately released and investigators haven't said how the person died.

The fast-moving wildfire was reported at midday Monday and spread quickly amid dry, windy weather.

Authorities ordered residents of more than 900 homes to evacuate.

"We're in a defensive mode, structure protection only," Jefferson County Sheriff's spokeswoman Jacki Kelley said. "We're not really fighting the fire right now."

Kelley said authorities don't know how many houses were lost but said it was at least five and could be more than 20, The Denver Post reported.

Fire personnel from 30 engine companies and different agencies were assembled to battle the blaze, authorities said.

There were no other reports of injuries, but a sheriff's deputy who was alerting residents to leave was trapped in his patrol car after he inadvertently drove into a ditch in the thick smoke, Kelley said. He summoned help by radio.

Single-digit humidity values, winds blowing at 40 to 50 miles per hour and a lack of snowfall during the past month put most of eastern Colorado under a red-flag warning for high fire danger on Monday, the National Weather Service said.

The high winds also had prompted flight delays at Denver International Airport.

Steve Segin, spokesman for the Rocky Mountain Coordination Center, said air tankers had been on alert for the past week in anticipation of extreme wildfire hazards in Colorado, but the gusty winds had grounded the fleet.

"There is really not much that can be done from the air until the winds subside," he said.

Up to a dozen smaller fires were reported from the northeast Colorado plains to the southern part of the state. There were no immediate reports of injuries or structures destroyed in any of the other fires.

Officials were calling in help from as far away as Arizona and hoped to be able to have air tankers drop fire retardant on the flames Tuesday.

Male nightingales that sing to defend their territory quarrel more at eye level, say scientists.

During night-time singing bouts, males are known to interrupt each others calls and rapidly change their songs.

Researchers wanted to investigate whether the birds tried to gain a height advantage by moving higher up in their chosen trees.

They found that the birds were actually more aggressive when singing from branches that were at the same height.

The findings, published in journal PLoS One, surprised the researchers.

"We expected nightingales singing from higher song posts to appear more threatening to their rivals," said Dr Valentin Amrhein from the University of Basel, who was the senior author of the study.

Alongside their colleagues from the Netherlands Institute of Ecology, the scientists theorised that the birds' positions could have a significant effect on how "threatening" their calls were.

Dr Amrhein explained that higher branches were thought only to be accessible to the fittest and most dominant birds, because taking up a loftier post increased threat of predators and reduced protection from the elements.

The team believed, therefore, that males singing from higher positions would represent more of a threat to their rivals, and elicit more of a defensive response from other males.

The team tested their idea by playing audio recordings of male nightingales at varying heights and monitoring how a real bird responded.

In the experiment, males responded more aggressively to calls made at the same height as them by increasing their "song rate". This meant that the birds took fewer pauses and changed their tune more often.

According to Dr Amrhein the results suggest that the birds have an advanced awareness of both their position and that of their rivals, and that they factor this information into their vocal performances.

"In future studies on vocal communication in birds, signalling height should be considered, because it apparently determines how birds perceive their rivals," he said.

Nightingales are found in tropical Africa and migrate to Europe, north west Africa, the Middle East and Central Asia to breed.

Males are famed for their nocturnal musical performances, which are made up of an impressive range of sounds including trills and whistles.

Females are thought to choose their mates based on the quality of these songs.

You can talk to fish and they don?t talk back. They can provide company without demanding it. They are fun to watch, and pretty, and easy to care for. And you don?t need to open any cans or clean any bowls of food ? all you need to do is sprinkle a few flakes of fish food twice a day to keep your fish from going hungry.

Unfortunately, fish aren?t too receptive ? if they are receptive at all ? to your presence. They don?t snuggle up against you, or keep your feet warm in winter. You can?t hug fish unless you want to squeeze them to death, nor can you play with them outside of tapping the glass. A recent study conducted at Marshall University may eventually help scientists develop new treatments for prostate cancer, the most common malignancy in American men.

The study, which focused on the effects of cadmium on the prostate, was conducted by Dr. Pier Paolo Claudio, an associate professor in the Biomedical Sciences Graduate Program and Department of Biochemistry and Microbiology at the university's Joan C. Edwards School of Medicine, and an international team of colleagues from the University of L'Aquila and the National Cancer Institute in Italy, and the University of Colorado Denver and the National Institute of Environmental Health Sciences in the United States.

An extremely toxic metal found in industrial workplaces, cadmium is commonly used in electroplating and is a key component in batteries and some paints. It is also found in cigarettes and some food supplies.

According to Claudio, scientists believe the prostate may be a target for cancer caused by cadmium, although the underlying mechanisms have been unclear.

"In our study, we investigated the effects of cadmium exposure in normal and in tumor cells derived from human prostate tissue," he said. "We were able to demonstrate the molecular mechanisms cadmium uses to induce carcinogenesis in the prostate."

Claudio, who said he has spent the last 15 years conducting research to understand the crosstalk between the factors that contribute to cancer progression versus those that protect from it, says this study is important because once those molecular mechanisms are understood, new therapies can be tailored to treat prostate cancer.

He added, "The focus of work in our laboratory is to understand the molecular mechanisms governing malignant transformation in order to tailor novel therapeutic strategies. To effectively design novel biological drugs, a thorough understanding of the mechanism of cancer pathogenesis is required. Our study will contribute to the body of knowledge available to science and may lead to exciting new treatments for this common cancer."
